This form will only be used to collect billing information from certain U.S. residents whose State (New York) requires our organization to collect State and local sales tax for online purchases. Upon receiving this information, our staff will look up the correct State and local sales tax, then email final payment instructions from PayPal (our current e-commerce service provider) to complete the billing process.
